Partner	Provides CHM Donor Society membership benefits for up to two adults and the primary member's children or grandchildren under age 18.	300.00
Sponsor	Provides CHM Donor Society membership benefits for up to two adults and the primary member's children or grandchildren under age 18.	500.00
